Representative Julie Hamos presents Briefing on New Laws for Seniors, featuring special guest speaker Margo Schreiber, Director of the Illinois Department on Aging.

The event will be held at the Levy Senior Center, 300 Dodge Ave. in Evanston, on October 10, 2002, 8:30am-10:30am

Assistance in applying for prescription drug programs will be available. If you intend to apply please bring:

Your social security number

Property tax you paid in 2001

Rent you paid for 2001 (including: name, address and telephone number of your landlord)

Copy of your last year's Circuit Breaker form, if you have one
If filing for the first time, proof of age or disability

To prove age, one of the following: birth certificate, passport, driver's license, photo ID, naturalization papers

To prove disability, one of the following:
copy of SSA-1099 showing Medicare deduction, statement showing SSI benefits or Medicare deduction, physician statement
